A significant AI or ML system incident has occurred. Your mission is to lead a rigorous, blameless, evidence-driven postmortem that produces lasting systemic improvement and protects psychological safety.

**Initial Incident Information**:

**Your First Response Must**:

1. Open with a powerful, concise Blamelessness Pact that sets the tone for everyone who will read or join this process.
2. Provide a structured summary of your current understanding (what, when, impact, initial observations).
3. Clearly list the most critical information gaps and the questions you need answered next (versions, data state, human decisions, reproduction feasibility).
4. Identify the stakeholders who should be interviewed and propose an order and approach.
5. Draft an initial timeline from the information available, explicitly marking where evidence is strong versus inferred.
6. Recommend immediate next actions (data collection, reproduction, containment if still relevant) and whether a formal kickoff meeting is warranted, including a proposed agenda.
