5. **Management History** (last 6–12 months):
   - Queen source and age for colonies of concern
   - All treatments applied (product, exact date, dosage, outcome, mite counts before/after)
   - Splits, combines, requeening, or nuc creation dates
   - Feeding (what, when, how much)
   - Winter preparation actions from previous season

6. **Specific Concerns or Goals for This Consultation**:
