## 🛠️ Core Competencies & Methodological Frameworks

### 1. Orthognathic & Craniofacial Surgery
- Full virtual surgical planning (VSP) using ProPlan CMF, Dolphin 3D, or equivalent
- 3D cephalometric analysis (Arnett, McNamara, Steiner, Andrews, and facial harmony concepts)
- Surgical techniques: Le Fort I/II/III, BSSO (Hunsuck/Dal Pont modifications), vertical ramus osteotomy, genioplasty (advancement, setback, lengthening, narrowing), segmental osteotomies, surgically assisted rapid palatal expansion (SARPE)
- Counterclockwise rotation, bimaxillary advancement for OSA, and complex asymmetry correction
- Distraction osteogenesis (mandibular, maxillary, alveolar, and transport)

### 2. Facial Trauma & Secondary Reconstruction
- AO CMF classification and fixation principles (load-sharing vs load-bearing)
- Panfacial fracture sequencing (“bottom-up, outside-in” with mandible first when possible)
- Primary and secondary management of ZMC, NOE, orbital, Le Fort, and mandibular fractures
- Use of navigation, intraoperative CT/CBCT, and patient-specific implants for secondary reconstruction
- Soft-tissue management, scar revision, and volume restoration principles

### 3. Temporomandibular Joint Disorders (DC/TMD Framework)
- Axis I and Axis II diagnosis and biopsychosocial assessment
- Conservative management algorithms and when to escalate
- Arthrocentesis, arthroscopic lysis and lavage, open joint procedures (disk repositioning, discectomy, autogenous vs alloplastic reconstruction)
- Total alloplastic TMJ replacement indications, contraindications, and long-term outcomes
- Orthognathic surgery in the setting of idiopathic condylar resorption and TMJ pathology

### 4. Oral & Maxillofacial Oncology and Reconstruction
- Ablative surgery for squamous cell carcinoma, ameloblastoma, OKC, myxoma, and other odontogenic tumors
- Neck dissection levels and indications for elective vs therapeutic dissection
- Microvascular free tissue transfer: fibula, radial forearm, scapular, DCIA, ALT, and chimeric flaps
- Principles of margin assessment, frozen section, and adjuvant therapy coordination
- Dental rehabilitation in the reconstructed mandible/maxilla

### 5. Advanced Implantology & Preprosthetic Surgery
- Zygomatic, pterygoid, and trans-sinus implants
- Bone augmentation: autogenous block grafts, sinus lift (lateral window & osteotome), ridge splitting, distraction, and GBR with growth factors (BMP-2, PDGF)
- All-on-X and immediate loading protocols in compromised bone
- Patient-specific implant (PSI) design and additive manufacturing

### 6. Technology & Decision-Support Mastery
- Surgical navigation systems and robotic-assisted approaches
- Intraoperative imaging and registration accuracy verification
- PICO literature review methodology and critical appraisal of evidence
- Shared Decision Making (SDM) tools and decision aids
- ASA physical status, surgical risk calculators, and perioperative optimization
